Improving the Delivery of Adult Diabetes Care Through Integration: Sharing Experience and Learning

In October 2014 the charity Diabetes UK published a report titled "Improving the Delivery of Adult Diabetes Care Through Integration: Sharing Experience and Learning".  The report identifies five key enablers for integrated care and outlines specific local initiatives in the UK which deliver integrated care for patients with diabetes. For example, the Royal Wolverhampton Hospitals NHS Trust (which delivers community and hospital services in Wolverhampton) along with the local Clinical Commissioning Group and various GP practices have been involved in an integrated and patient-centred model of diabetes care for many years. The model is characterised by partnerships between primary and specialist care that aim to increase the proportion of services delivered through primary care. Voices on person-centred care

What is person-centred care to a patient, a physician, a nurse, a hospital head of department, an industry representative and a researcher? How do these people, who all have experience of receiving or working with person-centred care, see person-centred care from their perspectives? Short film clips from the University of Gothenburg Centre for Person-centred Care - GPCC. GPCC is an in terdisciplinary research centre, established in January 2010, with the support of the Swedish government's strategic investment in health and care research. Uniquely GPCC has evaluated the effects of person-centred care in controlled studies. This has established that person-centred care is capable of enhancing the efficiency of the care process. Apart from considerable monetary savings resulting from amongst other things shorter hospital stays, significant positive effects from a patient perspective have been seen, as care is adapted to each individual person and his or her resources are made use of.

Multi-specialty community providers and primary and acute care systems

The primary and acute care systems (PACS) model and multi-specialty community provider (MCP) model are particularly focussed on building partnerships between GP, hospital, community and mental health care organisations to build a health care system to meet their local population’s health needs.

This Conference will take a detailed look at each of these models of care. One year on from the launch of the PACS and MCP vanguard sites, it will provide an opportunity to hear from the sites and others on how are they working in practice with lessons for areas of the United Kingdom that are interested in implementing similar models for their local populations.

Primary care managers´perceptions of their capability in providing care planning with complex needs

The aim of this study is to investigate primary care managers´perceptions of their capability in providing care planning to patients with complex needs. Care planning is defined as a process where the patient, family and health professionals engage in dialogue about the patient´s care needs and plan care interventions together. 

Derivation and validation of the Personal Support Algorithm: an evidence-based framework to inform allocation of personal support services in home and community care

Personal support services enable many individuals to stay in their homes, but there are no standard ways to classify need for functional support in home and community care settings. The goal of this proyect was to develop and evidence-based clinical tool to inform service planning while allowing for flexibility in care coordinator judgment in response to patient and family circumstances. 

An Evaluation of the Role of an Intermediate Care Facility in the Continuum of Care in Western Cape, South Africa

A comprehensive primary healthcare (PHC) approach requires clear referral and continuity of care pathways. South Africa is a lower- middle income country (LMIC) that lacks data on the role of intermediate care (IC) services in the health system. This study described the model of service provision at one facility in Cape Town, including reason for admission, the mix of services and skills provided and needed, patient satisfaction, patient outcome and articulation with other services across the spectrum of care.

Nurses' experiences of person-centred care planning using video-conferencing

The aim was to illuminate how nurses experience person-centred care planning using video conferencing upon hospital discharge of frail older persons. This study provides valuable insights into challenges associated with care planning via video conferencing. The meeting format, that is video conferencing, is perceived as a barrier that makes the interaction challenging. Shortcomings in video technology make a person-centred approach difficult. The person-centred approach is also difficult for nurses to maintain when the older person or relatives are not involved in the planning.
